Membership Terms
Please review the plan terms before enrolling. Healthy Smiles Club is an in-office discount plan, not dental insurance.

- This is an in-office discount plan for Dental House VA, not dental insurance.
- The plan cannot be combined with another dental plan, discount, or promotion.
- Enrollment fees must be paid in full before the first cleaning to receive discounts.
- All cleanings must be used within one year or benefits expire.
- All treatment must be completed during the active membership period to receive discounts.
- Payments are non-refundable. Refunds are not issued for unused benefits or for obtaining dental insurance after enrollment.
- The calendar-year plan is prorated after July 1.**
- Plans automatically renew January 1 unless written notice is provided at least 30 days beforehand.
- Missed appointments may result in cancellation fees. More than two missed appointments may result in removal from the plan without a refund.
